A 24-hour voicemail is available for your use.  If you call outside business hours, please leave your name and a number where you may be reached during regular office hours.

The Intake Investigator will need the following information from you:

• Your name, address and phone number
• Child(ren)'s name and date of birth
• Child(ren)'s county of residence
• Parents' names and dates of birth
• Caseworker's name(s)
• Names of other agency officials you have spoken to
• Brief description of the problem
• Action(s) you are requesting from the OCO
  

The Ombudsman will determine if the OCO has the authority to investigate your complaint. You will receive an acknowledgment letter from the OCO providing referral information to assist you in resolving your problem, or notice that your case will be investigated.
