| How to Contact the Office of Children's Ombudsman |

Note: The OCO is not an emergency intervention service. If you are calling about a case in need of immediate action, call your local FIA office or law enforcement agency.
All contact with the OCO is strictly confidential

A 24-hour voice mail is available for your use. If you call outside business hours, please leave your name and a number where you may be reached during regular office hours.

| How to File a Complaint |

Contact the Office of Children's Ombudsman at (800) 642-4326 (MICH-FAM) or (517) 373-3077. The Intake Investigator will need the following information:
- Your name, address and phone number
- Child(ren)'s name and date of birth
- Child(ren)' county of residence
- Parents' names and dates of birth
- Caseworker's name(s)
- Names of other agency officials you have spoken to
- Brief description of the problem
- Action(s) you are requesting from OCO
The Ombudsman will determine if OCO has the authority to investigate your particular complaint. You will receive an acknowledgment letter from the Ombudsman providing referral information to assist you in resolving your problem, or notice that your case will be investigated.
