How to Apply

miAccount - Apply for Retirement

miAccount

miAccount is a fast and easy way to apply for retirement— five simple steps and you're done! Information about insurance, taxes, and electronic fund transfers (direct deposit) is provided to you during the online application process. We recommend that you submit your completed retirement application and mail photocopies of proofs ORS at least three months before your retirement effective date. You will not get retroactive benefits if you file more than 12 months after you become eligible for pension payments.

What you will need.

ORS cannot process your retirement until you complete all of the steps in miAccount and submit all of the required documentation. Make sure all forms requiring notarization are signed in the presence of a notary public. You must also sign the Retirement Checklist page and return it to ORS along with all required documents.

You'll also need to provide the following:

 

Forms When Required / Special Notes
HMO Application Required if you are enrolling in one of the HMOs. Contact the HMO for an enrollment form and submit it along with your proofs and miAccount documents.
Documentation ( do not mail in original documents) When Required / Special Notes
Proof of Marriage To name your spouse as a pension beneficiary and/or enroll him or her in your insurances, you must provide a copy of your marriage certificate.
Proof of Age - Self Required. Submit a copy of your birth certificate. If that's not available, click here to see other acceptable proofs of age.
Proof of Age - Pension Beneficiary Required if you selected a survivor option. Submit a copy of your pension beneficiary's birth certificate. If that's not available, click here to see other acceptable proofs of age.
Proof of Relationship - Pension Beneficiary Required if you are naming your child, sibling, or parent as your pension beneficiary. Acceptable proof: photocopy of the child or siblings birth certificate (or your own birth certificate if you're naming your parent).
Proof of Dependency - Insurance Dependents Required if you are naming an insurance dependent. Acceptable proof: Photocopy of the dependents' birth certificates.

If your insurance dependent is age 19 or older, acceptable proofs are your most recent 1040 tax form listing the dependent. (For children age 24 or older, provide the child's most recent tax return or a signed statement declaring his or her current year expected income.)

Proof of Insurance - Insurance Enrollee For anyone covered under another plan, including Medicare, who is enrolling in health insurance, include that plan's information as requested in the insurance application section of miAccount.
Proof of School Attendance - Insurance Dependents Required if your insurance dependent is a full-time student age 19 or older. Acceptable proofs: a student transcript or tuition receipt showing he or she meets the school's standards for full-time attendance.
Proof of Disability - Insurance Dependents Required if your insurance dependent is a disabled child age 19 or older. Acceptable proofs: a statement on the attending physician's letterhead certifying the child is disabled and incapable of self-sustaining employment.
Proof of Legal Guardianship - Insurance Dependents Required if your insurance dependent is a child for whom you are legal guardian. Acceptable proofs: legal guardianship papers issued by a court.
Social Security Statement Required if you selected an equated plan. To obtain your estimate, you'll need to visit your local Social Security Administration office and ask for documentation of your age 62 benefit amount.
Proof of Residency To cover your parent(s) or parent(s)-in-law, you must provide a copy of one of the following in the parent or parent-in-law's name showing your address: 1099 or 1098 tax form for previous year, voter registration card, state driver's license, or state issued ID.
