The UCC11 form is the approved form to request information and/or copies. State the debtor name you wish to search. Debtor names will be searched exactly as they are provided by the requestor. Also state whether you need an information listing of financing statements and tax liens, copies of the documents or both. The information listing does not provide information about collateral shown on financing statements.
