Requesting Another Person's Record

To request a copy of another person's driving or vehicle record, you must complete a record request form.  You must select one of the permissible reasons in Section 4 of the "Record Lookup Request" (BDVR-154) form that pertains to your request and give an explanation in Section 6 stating the reason you need the records.  Please follow the instructions.  If you need assistance in completing the form, call (517) 322-1624.

If you anticipate a high volume of requests (15 or more records per month), call (517) 373-8730 or toll free at (877) 675-6708 to establish an account with the Department of State.

Mail:  Mail the completed form and payment to:

Record Lookup Unit

Lansing, MI  48918-1502

Fax:  You must have a Department of State account or pay with Discover, Visa or MasterCard.  Fax the completed form to (517) 322-1181.

Walk-In:  Information for Walk-Ins.

Note:  Mailed or faxed requests are typically processed within three weeks, but can take up to eight weeks.

Customers without a permissible purpose will have personal information redacted from the record lookup response.
