Statewide Look-up: Please remember that the Statewide Look-up is a secure site.  For that reason, we ask that you do not share usernames and passwords.  If you have not accessed the statewide lookup recently, it is recommended that you access the site to ensure that your username and password are still recognized.  For security purposes, usernames and passwords that are not used over a 3-month period are  automatically deactivated.  If you need to have your username and password reinstated, please contact the QVF Help Desk at 1-800-310-5697.
