Plan review of proposed location, and equipment for a facility that will have storage tanks containing flammable or combustible liquid must be filed with the Michigan Department of Environmental Quality, Waste and Hazardous Material Division, Storage Tank Unit prior to commencing construction of the facility. The form to be completed and sent to the Storage Tank Unit, with a site map indicating the location of the tanks, dispensers, and water wells, for underground storage tanks is form EQP-3820, for aboveground storage tanks the form is EQP-3859. An inspection of the facility should be scheduled with the District Hazardous Material Storage Inspector. The inspector's name and contact information will be furnished on the Report generated following review of the application. Following the inspection a registration form must be completed and submitted to the Storage Tank Unit. A registration will be mailed to the facility indicating compliance with Michigan Rules.
