    Ambulance Service

    Required State License(s):

    Ambulances should be registered with the Department of Community Health. This includes "air ambulances" (helicopters, planes) and water craft transporting persons for emergency care. Contact Department of Community Health, Emergency Medical Services; (517) 241-3025.  

    Further Information:

    Non-emergency vehicles (such as vans taking patients to doctors appointments, etc.) may register with the Department of Community Health, but it is not required if they will not be transporting emergency patients. The company should write a letter to the Michigan Department of Community Health expressing their interest in registering their vehicles.  

    Non-emergency care vehicles (such as automobiles, vans or cutaway style vehicles taking patients to doctor appointments, etc. in exchange for compensation) must obtain a business license from the Michigan Department of Transportation, Multi-Modal Transportation Services Bureau, but it is NOT REQUIRED if transporting EMERGENCY care patients (See ambulances transporting persons for emergency care above).

    Regardless of the vehicle seating capacity, please call (517) 241-0679 or (517) 241-0680, OR obtain printable/fillable license application forms and other information on-line at www.michigan.gov/mdotptd and click on Bus & Limousine.

    Contact Blue Cross/Blue Shield in Detroit for information on enrolling as a Medicare provider.  

    When a business wishes to qualify for payment from Medicare, call
    (800) 633-4227; to enroll for Medicaid payment call (517) 335-5492.
