    How do I submit my Employer's Quarterly Tax and/or Payroll Report?

    (Formerly UIA 1020 Employers Quarterly Tax Report, UIA 1017 Wage Detail Report, and UIA 1020-R, Reimbursing Employer's Quarterly Payroll Report)

    Form UIA 1028, Employer's Quarterly Wage/Tax Report 

    Each calendar quarter, the UIA mails every contributing employer Form UIA 1028, Employer's Quarterly Wage/Tax Report. Wage detail information must be provided for every covered employee to whom wages were paid during the calendar quarter. This form is mailed about 30 days before the completed form is due back to UIA. However, if you do not receive Form UIA 1028, it is the employer's responsibility to file a timely report. You can obtain a blank copy of Form UIA 1028 from UIA's web site at www.michigan.gov/uia, via fax by calling the UIA's Office of Employer Ombudsman (OEO) at 1-855-4UIAOEO (855-484-2636). 

    Form UIA 1028 can also be filed online by selecting the Operate a Business option from the Michigan Business One Stop web site and selecting the task File and Pay Quarterly Taxes. Selecting this task will link you to the online services offered by UIA through the Michigan Web Account Manager (MiWAM). If you do not currently have a MiWAM account you can register for one and open the door to the many online services offered by UIA.

    Form UIA 1028 must be received by the UIA or filed through MiWAM on or before the 25th day of the month following the end of the calendar quarter. A report is due for each calendar quarter even if you had no covered workers, paid no wages during the quarter, or had no taxes due for the quarter. The due dates are:

    First Quarter - April 25

    Second Quarter - July 25

    Third Quarter - October 25

    Fourth Quarter - January 25

    UIA Electronic Reminders of Statement and Payment Due Dates

    Prior to the quarterly due date of the UIA 1028, Employer's Quarterly Wage/Tax Report, the employer can receive an email reminder.

    An employer can opt-in to receive an email reminder 10 days or 25 days prior to the quarterly due date.  To opt in, go online by selecting the Operate a Business option from the Michigan Business One Stop web site and selecting the Electronic Reminders of Statement and Payment Due Dates. Selecting this task will link you to the online services offered by UIA through the Michigan Web Account Manager (MiWAM). If you do not currently have a MiWAM account you can register for one and open the door to the many online services offered by UIA.
