The Michigan
Department of Licensing and Regulatory Affairs, Bureau of Health Systems has an
agreement with the U.S. Department of Health and Human Services (HHS) to assist
in determining whether health care programs are operating and remaining in
compliance with the federal Medicare Conditions of Participation to qualify for
Medicare payments. This includes the requirements for reimbursement, including
financial solvency; and the requirements of Title VI of the Civil Rights Act of
1964. There are no licensure fees.

D. SUMMARY OF LICENSE/APPROVAL PROCESS:
1.
Applicability (activities that require the license)
To serve patients that have been diagnosed with a terminal illness and
choose to receive hospice care.
2.
Pre-Application Requirements
Medical staff scheduled to start, admission materials, key policies,
inpatient care contract in place.
3.
Application Submission Requirements
BHS-LC-107 completed.
4.
Procedures and Time-Frame for Obtaining Permit or Approval
Contact the Department of Licensing and Regulatory Affairs, Bureau of Health Systems,
Division of Licensing & Certification at (517) 241-3830 to obtain an
application for a new Hospice program. A pre-licensure survey is conducted 1-2
weeks before a facility is ready to see patients.

Operational Requirements
Patient care plan directed by licensed physicians/nurses and license.
