Hospital LicenseAgency: Licensing and Regulatory AffairsThe Michigan Department of Licensing and Regulatory Affairs, Bureau of Health Care Services licensed Michigan hospitals are defined by their participation in Medicare. Each designation carries special operating requirements and/or restrictions. These licensed hospitals must comply with procedures, building codes and fire safety regulations from the Michigan Department of Licensing and Regulatory Affairs (LARA), Bureau of Health Care Services, Heatlh Facilities Division and the Department of Licensing and Regulatory Affairs, Bureau of Fire Services, respectively. Health Facility License -- Hospital B. STATUTORY AUTHORITY: Public Act 368 of 1978, as amended C. APPLICABLE REGULATION: PHC & R325.1001-1100 1. Applicability (activities that require the license)Patients under a physician's care requiring 24-hour acute care services. 2. Pre-Application RequirementsMedical staff scheduled to start, patient services (food, linen, cleaning, etc) scheduled. 3. Application Submission RequirementsHealth Facility/Agency Licensure Application (BHCS-HFD-100) 4. Procedures and Time-Frame for Obtaining Permit or Approval Pre-licensure survey conducted before patients can be seen. 1-2 weeks after facility is ready for occupancy. 5.
