    Mammography Machine Registration

    The Michigan Department of Licensing and Regulatory Affairs, MIOSHA issues authorization for radiation machines to be used for mammography.


    A. NAME OF LICENSE OR APPROVAL:

    Mammography Machine Authorization

    B. STATUTORY AUTHORITY:

    Part 135 - Radiation Control of the Public Health Code, Public Act 368 of 1978, as amended.

    C. APPLICABLE REGULATION: R325.5601 to R325.5698

    Ionizing Radiation Rules, Part 14 Mammography (R 325.5601 to 325.5698)

    D. SUMMARY OF LICENSE/APPROVAL PROCESS:

     1.  Applicability (activities that require the license)

    A person shall not use a radiation machine to perform mammography unless the radiation machine is registered with the Department and is specifically authorized to perform mammography.

     2.  Pre-Application Requirements

    None. However, the machine must be registered and a radiation shielding plan review must be completed prior to issuance of the mammography authorization.

     3.  Application Submission Requirements

    An application for mammography authorization must be on forms furnished by the Department and contain all information requested on that form.

     4.  Procedures and Time-Frame for Obtaining Permit or Approval

    Application for mammography authorization can be accomplished by calling the Radiation Safety Section at (517) 636-6800 to request an application. Once a completed application is received, authorization is completed within 10 business days. If additional time is needed to provide evidence of accreditation by the American College of Radiology, the Department may issue a temporary mammography authorization for up to six months.

     5.  Operational Requirements

    A user of a mammography machine must apply for authorization and receive the authorization prior to using the mammography machine.

     6.  Fees

    None if the application can demonstrate American College of Radiology mammography accreditation.

     7.  Appeal Process

    In a contested case, the Department shall conduct a hearing as provided in the Administrative Procedures Act of 1969.
