Any motor bus carrier that changes their business address

Application Submission Requirements
The applicant must completely fill out the Motor Bus Address Change Request
on Form 3089 http://mdotwas1.mdot.state.mi.us/public/webforms/public/3089.pdf.
Following completion of this step a new address on your Certificate of
Authority will be issued.

Procedures and Time-Frame for Obtaining Permit or Approval
Processing of a completed request usually takes from 1 to 2 weeks.
