If your worksite would like to receive an aggregate report on the health of your employee population, email us at demitzc@michigan.gov or call us at 517/335-8771. You will be assigned a worksite-specific code that you will share with your employees. Your employees should be instructed to enter the code when they complete their HRA registration. Your worksite-specific code will allow you to request an aggregate group report based on the health risk appraisals completed by employees that used the code when they registered. Unfortunately, we are unable to combine the results from multiple worksite setting locations.

Your worksite's aggregate data will not be associated with its name or with any other identifying information unless written permission is obtained. Occasionally, a worksite's aggregate data may be reported, but this information will not be associated with your worksite's name or other identifying information. Michigan Department of Community Health will receive aggregate data of all worksites that participate on a monthly basis; worksites' individual aggregate data will not be identifiable.
