| One of the initiatives that the Healthcare Group is supporting
is a health risk appraisal (HRA). The HRA is a scientifically valid tool
that provides individuals with reports on their health risks, which are
based on their answers to the HRA questions. The appraisal takes approximately
10 minutes to complete, and all individual information is kept confidential.
Michigan healthcare professionals can suggest that individuals
complete an HRA. Directions for completing the Health Risk Appraisal (HRA) can be provided to your patients. Individuals
can print two reports: one for their record and one to give to their healthcare
provider during their next visit. Advice and links to general health information
are also provided along with the individual report.
If you are a healthcare professional, you can request
a group number for your healthcare setting that will allow you to receive
group reports on the overall health status of your patients. If you need a Group number, read our "How
to obtain a Group number" page. Again, the individual's information is confidential; healthcare
professionals will only be able to receive a group aggregate report, and
not specific information on an individual.
