| How to obtain a Group number |

If your healthcare setting would like to receive a report that provides a general
overview of the health of your patient population, email us at theisenv@michigan.gov
or call us at 517/335-8754. You will be given a four-digit code that will be assigned
to your healthcare setting. You must provide this code to all patients, and ask
that they enter the code when they complete their registration.
Your unique healthcare
setting code will allow us to provide a group report based on the health risk
appraisals completed by patients that use that code when they register. Unfortunately,
we are unable to combine the results from multiple healthcare setting locations. Contact us if you have questions.
Contact us BEFORE patients begin completing the health risk appraisal.
