In this booklet, the forms that have been designed by the state for use in its work with the opt-out counties are being provided. We also are including sample petitions and reports, and the DNR has even offered its scripts and sample postings for use in site visits. These materials all are available for use or modification by opt-in counties, and we encourage these counties to take advantage of them.
