Finally, select "Online Qualifying Statement" from the list.
To complete and submit the qualifying statement online, follow these steps:
At the "Municipal Finance – Log On" screen do the
following:
Enter the 6-digit "Municipality Code" (do not use spaces,
hyphens, or commas). Note: With some municipalities, it is necessary to add zeros to
reach the required 6 digits. Please contact the Division with any
questions.
Enter the "Password" (do not use spaces, hyphens, or commas).
Enter the "Fiscal Year" (200X) for which you are submitting
the qualifying statement.
Select, as applicable, either "New" or
"Reconsideration."
Review for accuracy then click on "Submit."
Review the "Municipality Information" for accuracy. Do
not alter any of the information already contained in these fields. Instead,
note any necessary changes in the "Comments Box."
Enter the appropriate responses to the qualifying statement questions. Any
necessary comments or explanations should be noted in the "Comments
Box" that appears at the end of the questions.
Enter the general contact information (name, title, address, etc.).
Carefully review the qualifying statement for accuracy and completeness.
Select the "Submit" button.
If the qualifying statement was complete and properly received, a
verification notice "Statement Received" will appear in red at
the TOP of the screen.
If this message does not appear, scroll down and review the entire form
for missing information. Missing information will be indicated in red.
Enter this necessary information and re-select "Submit."
For additional verification and status updates, follow these steps:
Repeat steps 1, 2, and 3 as they appear above and then select
"Qualifying Statement Status" from the list.
Next, enter the appropriate fiscal year and select the county in which the
municipality is situated.
Scroll down until the desired municipality is located. A newly submitted
and received qualifying statement will appear as status "Pending."
