  Payroll Direct Deposit
When do I use the Payroll Direct Deposit Authorization form instead of MI HR Self-Service?
 
Answer:
If your financial institution requires authorization for withdrawal of funds from a joint account by someone other than or in addition to you, then you must complete and submit a Payroll Direct Deposit Authorization form to MI HR Service Center instead of using MI HR Self-Service or if you do not have access to MI HR Self-Service. Please remember to obtain and attach all required signatures to the form before submitting it.
