How do I change my address?
 
Answer:
Address changes are made by completing form UIA 1025, Employer's Request for Address/Name Change to the Unemployment Insurance Agency. The form can be submitted on-line through the B2G on-line services, via fax at 313/456-2130 or mailed directly to the Unemployment Insurance Agency Tax Office at PO Box 8066, Royal Oak, MI  48086-8068. Make sure to include your 7 digit UIA employer account number on the form.

Forms are also available by automated fax 24/7 by calling 1-800-638-3994 and selecting option #4.

Forms can be mailed to you by calling 313/456-2180.
