Use of Social Security Numbers When Filing Forms with the Workers' Compensation Agency

We are receiving an increasing number of forms in the agency without a complete 9-digit social security number.  While we appreciate the effort to protect claimant privacy, please be aware that our agency is allowed under the Social Security Number Privacy Act to require this information as a means to identify injured workers on our automated system.  Care should be taken to ensure that the number is not visible through the envelope window or outside the envelope or package.

All forms received without a full 9-digit social security number will be returned.  Thank you.
