The Michigan Office of Financial and Insurance Services (OFIS), Department of Consumer & Industry Services has in place a health insurance complaint procedure with regard to problems individuals may have with health care service.

The Health Insurance & HMO Grievance Procedure can be used if a health care insurer is refusing to treat a client while their workers' compensation claim is in dispute. The Health Care - Request for External Review Form (Form 0018) is used to begin the External Review process by OFIS.
