Change to Agency Online Fill-In Forms

Effective immediately, all WCA "extended document" pdf forms previously available on our website have been removed. This is the feature that enables users of the forms to save a completed version of the document when the form is used with Adobe Reader. All forms have been replaced with pdf versions that do not have the extended use features. All forms will still be in fill-in form but completed forms cannot be saved if used in conjunction with Adobe Reader.

Please immediately stop using any previously down-loaded extended document versions. We apologize for any inconvenience this action may cause.
