Supplier

  • Supplier Information

    A supplier is licensed with the bureau to rent, sell, or lease equipment or to sell charity game tickets to licensed organizations. Supplier licenses are regulated under Act 382 of 1972 and the Supplier administrative rules. (Both are listed under Act and Rules.)

    The Supplier Application below includes instructions. Complete application and mail it to the bureau with a $300 annual license fee and all required documentation. Applicants are given a criminal background check. The supplier license expires on September 30 of each year.

    Should the applicant wish to sell charity game tickets, they must first contact the ticket specialist as indicated on the application instructions. Additionally, a performance bond must be posted at an amount determined by the bureau that is not less than $50,000 and not greater than $1,000,000.

    A supplier or person residing in the same household as the supplier shall not be involved directly or indirectly with the rental or leasing of a facility used for an event.
