I have an active patient registry identification card and I recently divorced/married/changed my name. How do I update my name on my registry card? How much is the fee?

Prior to your renewal period (more than 90 days before your card is due to expire):
You may change your name online or submit a paper form.

To change your name online, you must be a patient, register for an online account and link your existing record.  Our online amendments are available on our website at www.michigan.gov/mmp.

To change your name by mail, you must complete the Change Name or Address Form. The form includes instructions and lists the supporting documentation you must submit with the form. The form can be found on our website at www.michigan.gov/mmp.

During your renewal period (less than 90 days before your card is due to expire):

You can make changes to your name during the renewal of your registry card.

You may renew online or submit a paper application.  To renew online you must be a patient without a caregiver whose physician has an online account.  You must still visit your physician for online physician certification.  Online instructions and the online portal are available on our website at www.michigan.gov/mmp.  When entering your information, enter your new name in the appropriate fields. 

To mail your renewal, submit an application with the required supporting documents and documentation of your legal name change. The MMMP Application Packet can be found on our website at www.michigan.gov/mmp.
