I have an active patient registry identification card and I recently moved. How do I update my address on my registry card? How much is the fee?

Prior to your renewal period (more than 90 days before your card is due to expire):
You must complete a change of address.  You may do this online or submit a paper form.

To change your address online, you must be a patient, register for an online account, and link to your existing record.  Our online amendments are available on our website at www.michigan.gov/mmp

To change your address by mail, you must complete the Change Name or Address Form. The form includes instructions and lists the supporting documentation you must submit with the form. The form can be found on our website at www.michigan.gov/mmp.

During your renewal period (less than 90 days before your card is due to expire):
You can make changes to your address during the renewal of your registry card. 

You may renew online or submit a paper application.  To renew online you must be a qualified patient without a caregiver and your physician must have an online account. You must still visit your physician for online physician certification.  Online instructions and the online portal are available on our website at www.michigan.gov/mmp. When entering your information enter your new mailing address in the mailing address field. 

To mail your renewal, the MMMP Application Packet can be found on our website at www.michigan.gov/mmp. When filling out the application, enter your new address on the address line.
