Document Requests - Certification Letter, Duplicate License, License History

Licensees are encouraged to utilize the Change button on the National Insurance Producer Registry (NIPR) web page to verify or change their address, phone, or email.

Certification Letter - Some states require a Certification Letter as part of the application process for Bail Bonds producers, Surplus Lines Non-Resident licensure, and for Third Party Administrators. To request a Certification Letter, submit an email request to DIFS-Licensing@michigan.gov using Certification Letter as the subject line. In the body of the email, provide the name as it appears on the insurance license, and the 7-digit Michigan System ID number.

Duplicate License -To request a duplicate license, submit an email request to DIFS-Licensing@michigan.gov using Duplicate License as the subject line. In the body of the email, provide the licensee’s name as it appears on the insurance license, and the 7-digit Michigan System ID number. The duplicate license will be mailed to the Mailing Address currently on file for your Michigan license.

License History - If a license history from the DIFS database is required instead of a print-out from the DIFS Insurance Licensee Locators, submit an email request to DIFS-Licensing@michigan.gov using License History as the subject line. In the body of the email, provide the name as it appears on the insurance license, and the 7-digit Michigan System ID number.

Licensees who need to inactivate their insurance license can refer to the Termination of Insurance Licenses web page for the appropriate process.
