Executive Rule 11(1) requires employers to maintain a record of the daily screening. What is an acceptable "record"?

To satisfy the requirement to maintain a record of the daily screening for employees and contractors, an employer’s options include:

  • Maintaining copies of the completed questionnaires.
  • Maintaining a log/report about the daily questionnaires. Minimally the log/report would include date, employee/contractor identifier, and pass/fail indication.
