If you add new dependents to your insurance coverage during BOE Part 1, you must mail or fax the appropriate proof of eligibility documentation1 to the MI HR Service Center2 by September 4, 2020 for the enrollment to be valid. Click the links below to view a listing of valid documents and requirements.
State Police Employees Only: You must mail or fax the Evidence of Insurability form and Life Insurance Enrollment form to the MI HR Service Center no later than August 27, 2020.
