Welcome to the Primary Care and Public Health Web Page
While public health focuses on the health of groups of people – often defined by shared geography, demographics or a common medical condition - primary care has traditionally focused on the health of individuals, providing preventive care, and treatment for acute and chronic conditions. Together, public health and primary care share the goal of helping people achieve optimal health. The Michigan Department of Health and Human Services (MDHHS) is committed to providing leadership and vision for collaboration between public health and primary care, along with practical tools and resources. This site provides resources health care partners can use to improve patient care and outcomes.
