Clare Pharmacist Loses License

The Michigan Department of Community Health (MDCH) has summarily suspended the pharmacist license of Douglas W. Bouchey, R.Ph. The MDCH Bureau of Health Professions orders an immediate suspension of a license when it believes that the public’s health, safety, and welfare warrant emergency action.

On November 17, 2003, Bouchey’s Iowa pharmacist license was suspended based on Bouchey's illegal dispensing of controlled substances/prescription medications and dispensing controlled substances/prescription medications without complete patient information. Bouchey voluntarily surrendered his Iowa pharmacist license on March 16, 2004.

On June 17, 2004, MDCH issued an order summarily suspending Bouchey’s pharmacist license. An administrative hearing will be scheduled to address the status of Bouchey’s license.
