VIS Information for SPIKEVAX
Because the Moderna COVID-19 vaccine for 12 years and older (SPIKEVAX) has been approved for a Biologics License Application (BLA), a vaccine information statement (VIS) must be provided to patients when SPIKEVAX (2024-2025 formula) is administered to this age group.
Note: The 2023-2024 formula vaccine is no longer authorized for use in the United States. VIS sheets for the 2024-2025 formula will be posted here when they are updated by the CDC.
