March 10, 2023: Attention All Providers: The purpose of this notice is to offer guidance regarding when to complete the checklist questions in the CHAMPS provider enrollment steps. Providers do not need to complete the checklist questions and submit them when they are incomplete and that is the only step needing to be completed. The checklist question step will always display as incomplete. Any time an update is made to any of the CHAMPS enrollment steps, providers will be required to complete the checklist questions prior to submitting the provider enrollment application or modification.
