Apply for Funding

Instructions on how to submit an application to the Healthy Community Zones grant program are included below. Applicants are encouraged to complete the grant application in advance of the application deadline to allow enough time to finish the entire application process and to receive technical assistance, if necessary.
 

  • Submit a Proposal

    To apply to the Request for Proposals (RFP), visit the EGrAMS website.* On the left panel, under the “Current Grants” section, click “Public Health Administration.” Select “HCZ-2025” from the list of grant programs.

    For more information, visit the EGrAMS website and select the "About EGrAMS" link in the left panel to access the "Competitive Application Instructions" training manual.

    Recording of Mid-Point Application Conference Webinar

  • How to Apply for Healthy Community Zones Funding

    All applicants must have a State of Michigan EGrAMS (Electronic Grants Administration & Management System) account. MDHHS utilizes the EGrAMS software to implement its outgoing grant agreements. To create an account, visit https://egrams-mi.com/mdhhs.
     

    Eligible Applicants

    Funding is open to local public, private or nonprofit 501(c)(3) organizations, including organizations supporting tribes and people with disabilities. Partnerships and collaborations are encouraged. Only one application may be submitted by the collaborative coordinator. Applicants are encouraged to apply with a trusted partner as a fiduciary.
     

    Scoring Criteria

    The total maximum number of points that an application can receive is 100 points. The maximum number of points for each of the categories is as follows:
