How do I submit deficiency documents/responses for my Annual Financial Statement (AFS) Report?

All deficiency documents/responses in regards to the medical AFS must be uploaded to the licensee's medical prequalified ERG record in Accela Citizens Access Portal (ACA) or sent via email to MRA-AFS@michigan.gov.

  • If uploading to the licensee's ERG record, use the "AFS Documents" document category.
  • If sending via email, include the licensee's name and ERG # in the subject line.
