Residency in a Disproportionately Impacted Community
When applying for a marijuana-related conviction fee reduction, an applicant may submit documents such as the Judgment of Sentence. Documents should include the applicant’s name, date of birth, the name of the conviction, and the date of the conviction. If the conviction has been expunged, an Order on Application to Set Aside Conviction may be accepted. For more information, please see the instructions linked above.
Registered Primary Caregiver for at least 2 years between 2008 and 2017
When applying for the caregiver fee reduction, the applicant will complete and notarize the form linked below. In submitting the form, the applicant will include a copy of their driver’s license or State ID card.
