Enroll in VA Health Care
The Veterans Health Administration is America's largest integrated health care system with more than 1,700 care sites, serving nearly 9 million veterans each year. VA health care services include inpatient, outpatient, pharmacy, dental care and others.
In Michigan, 25 facilities offer VA health care, including 15 Community Based Outpatient Clinics (CBOCs), five outpatient clinics and five major medical facilities.
Service members who separated under any condition other than dishonorable may qualify for VA health care benefits. Reservists and guardsmen who were ordered to active federal service and fulfilled their tour of duty may also be eligible.

To apply for VA health benefits, a 10-10EZ form must be submitted online, by phone, in person or by mail. A DD-214 is required.
- Online - Fill out the VA Form 10-10EZ, Application for Health Benefits online. A submission number will be issued once completed. Apply for Health Care | Veterans Affairs (va.gov)
- By phone - Call 877-222-VETS (877-222-8387) to complete the application. A VA Form 10-10EZ will be mailed for review, signature and return.
- In person - Visit a VA health care facility and complete VA Form 10-10EZ on-site. Locate the nearest facility at Find VA Locations | Veterans Affairs
- By mail - Download and complete a VA Form 10-10EZ, and return to:

Once an application is processed, the VA will send a letter with enrollment that will inform the veteran of their Priority Group assignment and copay status. The letter will also provide instructions on how to appeal the decision. Learn more about VA health benefits by visiting the VA website at VA Health Care | Veterans Affairs.
Once enrolled, most veterans will remain enrolled without further action on their part. However, certain veterans are required to provide income information to determine their priority level. These veterans will be mailed VA Form 10-10EZ to complete on an annual basis for re-enrollment. Failure to update this information could adversely affect a veteran's enrollment status.
