Dialysis (ESRD) Center

End stage renal disease (ESRD) or dialysis centers are not state licensed as a health facility or agency in Michigan. If a provider is interested in participating in the Medicare/Medicaid programs, the Department of Licensing & Regulatory Affairs, Bureau of Community & Health Systems does act as the State Agency (SA) on behalf of the US Centers for Medicare and Medicaid Services (CMS) to certify providers/suppliers.
