Amend a Previously Submitted Form UIA 1020 or UIA 1020-R
Debit (increase in tax liability) adjustments to previously filed Forms UIA 1020 (contributing employers) or Forms UIA 1020-R (reimbursing employers) can be submitted for the current and six previous calendar years.
Credit (decrease in tax liability) adjustments to previously filed Forms UIA 1020 (contributing employers) or Forms UIA 1020-R (reimbursing employers) can be submitted for the current and three previous calendar years from the date of first payment on the quarter being amended.
An amended report can be filed by completing Form UIA 1028 Employer's Quarterly Wage/Tax Report (for contributing and reimbursing employers) online by selecting the Operate a Business option from the Michigan Business One Stop web site and selecting the task Amend A Previously Submitted Form UIA 1028. Amend a Quarterly Wage Report
To correct a previously submitted Form UIA 1017, Wage Detail Report, use Form UIA 1028 Employer's Quarterly Wage/Tax Report. A separate Form UIA 1028 must be filed for each quarter being amended. The total wages reported on the original form, internet or magnetic tape filing must equal the total gross wages reported on Form UIA 1020, Employer's Quarterly Tax Report, filed by a contributing employer or Form UIA 1020-R, Reimbursing Employer's Quarterly Payroll Report, filed by a reimbursing employer..

Report a Change in Business using Form UIA 1772, Discontinuance or Transfer of Payroll or Assets in Whole or in Part
Form UIA 1772, Discontinuance or Transfer of Payroll or Assets in Whole or Part is required to be filed with UIA if any of the following occur:
1) Sale of business
2) Lease of business
3) Foreclosure
4) Merger
5) Reorganization
6) Bankruptcy
7) Dissolution or Discontinuance of Business
8) Death of Business Owner
9) New Partnership formed
10) Incorporation of a business
11) Cessation of employment
12) Employee Leasing or Professional Employer Organization (PEO) arrangement
13) A client entity has terminated its contract with an employee leasing company or a PEO
14) Any change in business entity

File a Power of Attorney
Any time you want to grant a Power of Attorney for your UIA account you can notify the Agency of this by either completing Form UIA 1488 by selecting the Operate a Business option from the Michigan Business One Stop web site and selecting the task Change of Address. Selecting this task will link you to the online services offered by UIA through the Michigan Web Account Manager (MiWAM).
If you do not currently have a MiWAM account you can register for one and open the door to the many online services offered by UIA.
An employer can either give General Authorization or Limited Authorization to a representative. General Authorization gives the representative the ability to:
1) Inspect or receive confidential information.
2) Represent you and make oral or written presentations of act and/or argument on your behalf.
3) Sign returns.
4) Enter into agreements.
5) Receive mail from UIA. This includes forms, billings, and notices. This authorization applies to all tax and non-tax matters and is for all years or time periods.
An employer can also grant Limited Authorization and then select which of the five functions above they wish to grant to the representative. Form UIA 1488 can also be used to change a Power of Attorney representation or to revoke a previous authorization.
