No initial offer payment, insufficient initial offer payment, incomplete application, open bankruptcy proceedings, failure to provide appropriate signature(s) on Form 5181, no offer amount on Form 5181, failure to respond to additional information request, computed or unfiled tax returns, deceased taxpayer, no debt assessed, ineligible tax type or the account was paid in full.
