Hazardous Waste (Biennial) Reporting

Hazardous Waste (Biennial) Report for Hazardous Waste Large Quantity Generators (LQGs) and
Permitted/Interim Status Treatment, Storage, or Disposal Facilities (TSDs)

Staff of the Michigan Department of Environmental Quality (MDEQ), Environmental Resource Management Division (RMD), Waste Tracking and Data Management Unit, will mail out a biennial report form packet, by the first week of February in even numbered years, to any site that had a site activity status recorded in the RMD Waste Data System (WDS) database for any month during the previous odd numbered year as a LQG or TSD.  Any site that was a was a LQG during the previous odd numbered year, even for one month, but that activity was not on record with RMD in the WDS database, the site is required to contact the WHMD (517-335-5035) and request a copy of the biennial report.

The biennial report packet is separate from the annual invoice packet that is also sent out to hazardous waste LQGs and permitted/interim status TSDs in even numbered years. For annual invoice packet sent out in odd numbered years, there is a slight difference in the packet contents:

Annual invoice packet covering activities/data in even numbered years - sent to hazardous waste LQGs, SQGs, used Oil processors, and hazardous waste permitted/interim status TSDs every year) - will include:

Biennial reporting packet covering activities/data in odd numbered years - sent out to the site by the 1st week of February in the even numbered years - will include:

Annual invoice packet covering activities/data in odd numbered years - sent out to the site by the end of February in the even numbered years - will include:

1. Site Identification Verification Form - EQP5150-v

2. On-site Waste Generation and Management Form

3. List of manifest copies received by MDEQ for hazardous waste where data is missing for waste:

 a. shipped off-site

 b. received on-site

3. List of manifest copies received by MDEQ for hazardous waste shipped off-site.

Note : this will include hazardous waste shipped off-site from the designated facility where they either rejected all or part of a shipment or there were residues.
