How to Apply for a Michigan Aircraft Dealer License

1. Obtain a Michigan sales tax license number.

2. Obtain a Michigan aircraft dealer license form, number 4021
3. The application package must contain:
   • a completed and signed form 4021, application for dealer license
   • a copy of your Michigan sales tax license, and
   • a check for $25 payable to the State of Michigan

The dealer license is valid for the calendar year.
