The Client Application Process explains what will happen when you apply for assistance, information you should bring with you, your rights and responsibilities, etc.
If you are already receiving help from the Department of Human Services (DHS) you must contact your specialist and complete the Application for State Emergency Relief (SER). All others must complete both the Assistance Application and the supplemental Application for SER. Applications are available below or from a local DHS office. Hand-deliver, mail or fax your application(s) to the DHS office.
