Information on IRS Form 1095-B for People with Health Care Coverage Provided by the State of Michigan

If you or someone in your household had health care coverage through a State of Michigan Medicaid program, you may receive Internal Revenue Service (IRS) Form 1095-B. This form provides information about the months you had Minimum Essential Coverage (MEC) under the Affordable Care Act.

You do not need a 1095-B Health Coverage form to file your taxes: **This form is informational only and does not need to be filled out or sent back to the IRS or the Michigan Department of Health and Human Services.** Per IRS.gov regarding the 1095-B, "...Taxpayers can prepare and file their returns using other information about their health insurance. You should not attach any of these forms to your tax return."

How to Get Your 1095-B Form (If Needed):

  • Online: Log in to the myHealthPortal to download and print your form (available for current beneficiaries) at www.michigan.gov/myhealthportal.
    • First time users of myHealthPortal will need to complete a brief registration.
    • Once logged in, go to the Communications menu, then click on letters, then select the 1095B if it's available.
    • Download or print the 1095-B form yourself.
  • By Phone: Call the Beneficiary Help Line at 1-800-642-3195 (TTY 1-866-501-5656) to request a copy by mail.

You will not have a 1095-B form if you have one of the following programs since they do not provide MEC:

    • Plan First!
    • Emergency Services Only (ESO)
    • Maternity Outpatient Medical Services (MOMS)
    • Ambulatory Prenatal Services (APS)
    • Coverage provided while you were incarcerated
    • Being on a deductible (spenddown)

What to Do if Something on the Form is Wrong:

Please note:  If your address is correct on your case today (ex. It is correct in MI Bridges) but was not correct on the form, a corrected 1095-B form is not required.

If the name, social security number (SSN), date of birth (only appears if the SSN is not populated) or months of health care coverage on the form is incorrect, call the Beneficiary Help Line to request an update.

Do NOT call the IRS or your Michigan Department of Health and Human Services (MDHHS) local office specialist if something on the form is wrong. They will not be able to update it.
