How to initially become licensed:

• Prelicensing (original, signed course completion certificate required for Accident & Health, Life, Property, Casualty, Personal Lines) Exam must be taken initially within six months from the certificate date of course completion.
  
  
• Application/License fee
  $10.00 check or money order payable to the State of Michigan. $110 if also applying for surplus lines license.
  
  
• FIS 0223 Fee Card if you apply using form FIS 0220 Application for Resident Individual Producer
  
  
• FIS 0225 (ONLINE) Application for Individual Producer License (bring confirmation number to exam)
  
  - OR -
  
  FIS 0220 (mail-in paper form) Application for Resident Individual Producer Insurance License. Application is good for six months from entry into our database. If exam is not passed within six months, individual must reschedule exam and submit new application and fees.
