Correct a Birth Record
| The following forms are used to correct a birth record or to add a father's name to a birth record. PHOTO IDENTIFICATION IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN BIRTH RECORD.
In order to make a correction or change to a Michigan birth record, you will need to submit an official application. The application must include the correct payment and in most situations appropriate supporting documentation. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation that is required, and the appropriate fee payment. This form is to be used to correct errors and omissions on a Michigan birth
record of a person any age. This form should also be used to correct file date issues, to record a court-ordered legal name change, change a child's name or mother's name as a result of the parents' marriage, or to remove or replace a father's name on a birth record.
