Correct a Death Record
| The following forms are used to correct a Michigan death record. Please review the following list of applications to correct or change a Michigan death record.
In order to make a correction to a Michigan death record, you will need to submit an official application. The application must include the correct payment and in most situations appropriate supporting documentation. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation that is required, and the appropriate fee payment. Please note that some
changes to Michigan death records cannot be initiated by submitting one of the above

If you need to correct a Michigan marriage record, please contact the Circuit Court in the county where the license was obtained.
