Correct a Birth Record

The following forms can be used to correct a birth record, add a father's name to a birth record, or to verify whether or not a change has been made to a birth record.

PHOTO IDENTIFICATION (OR THESE ALTERNATE FORMS OF ID) IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN BIRTH RECORD.

Application to Add a Father
Application to add a father to a birth record. This form is NOT to be used to remove or replace a father's name on a birth record.

Correct or Change a Michigan Birth Record - Any Age
Application to change or correct general errors or omissions on a Michigan birth record.

Verification of a Record Change to a Michigan Birth Record
Application to verify whether a change has been made to a Michigan birth record. If a change is found, you will receive a copy of the application and a letter from the Changes Unit. If a change is not found, you will receive a letter indicating that.
