Correct a Birth Record


CORRECT A BIRTH RECORD - FAQ'S

The following forms can be used to correct a birth record, add a father's name to a birth record, or to verify whether or not a change has been made to a birth record.

PHOTO IDENTIFICATION (OR THESE ALTERNATE FORMS OF ID) IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN BIRTH RECORD.


  • Application to Add a Father
    Application to add a father to a birth record. This form is NOT to be used to remove or replace a father's name on a birth record.

*** Please note, if you need to correct or change a record of a vital event that occurred in another state, you should contact that state's vital records office.

Please note that some changes to Michigan birth records cannot be initiated by submitting one of the above forms. If the above forms do not appear to relate to your needs, please call 517-335-8660 for assistance, Mon-Fri 8:00 am - 5:00 pm ET (except for State holidays).
