Correct a Death Record



The following forms are used to correct a Michigan death record or to to verify whether or not a change was made to a death record. Please review the following list of applications to correct or change a Michigan death record.

In order to make a correction to a Michigan death record, you will need to submit an official application. The application must include the correct payment and in most situations appropriate supporting documentation. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation that is required, and the appropriate fee payment. If you have questions as to which form to complete or how to complete or submit the form, please call our Changes Unit at 517-335-8660, Mon-Fri 8:00 am - 5:00 pm ET (except for State of Michigan holidays).

PHOTO IDENTIFICATION (OR THESE ALTERNATE FORMS OF ID) IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN DEATH RECORD.
