Correct a Death Record

The following forms are used to correct a Michigan death record or to to verify whether or not a change was made to a death record. Please review the following list of applications to correct or change a Michigan death record.

In order to make a correction to a Michigan death record, you will need to submit an official application. The application must include the correct payment and in most situations appropriate supporting documentation. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation that is required, and the appropriate fee payment. PHOTO IDENTIFICATION (OR THESE ALTERNATE FORMS OF ID) IS REQUIRED TO MAKE A CHANGE OR CORRECTION TO A MICHIGAN DEATH RECORD.

Correct a Michigan Death Record
Application for next-of-kin or funeral service licensee.
Verification of Record Change to a Michigan Death Record
This application form is to be used to verify whether a change was made to a Michigan death record. If a change is found, you will receive a copy of the application and a letter from the Changes Unit. If a change is not found, you will receive a letter indicating that.

Physician Application to Correct Michigan Death Record before 1/1/2004
This form to be used by the physician if death occurred PRIOR to January 1, 2004.

Physician Application to Correct Michigan Death Record AFTER 1/1/2004
This form to be used by the physician if death occurred AFTER January 1, 2004.
