How do I fill out and submit the Employment Verification Form?
The form helps us to verify your employment as a frontline worker between April 1 – June 30, 2020, so all of the responses should be able that time period specifically.
It can be completed either electronically (it is a fillable pdf form) or by printing and handwriting the responses. There are two parts to the form:
- Part I: should be completed by you.
- Part II: should be completed by an Authorized Person, this is either your supervisor or a Human Resources representative (from your employer between April 1 – June 30, 2020). If you were self-employed during this time, you should check the “self-employed” box and complete Part II yourself.
If you worked for more than one employer in any of the essential industries during April 1 – June 30, you’ll need to complete the form with each employer.
The completed form should be either:
- Emailed to MiStudentAid@Michigan.gov. If you don’t have access to a scanner, you can take a picture of the complete form and attach the image to the email.
- Mailed to: Students Scholarship & Grants, PO Box 30462, Lansing, MI 48909.
Please note that there is no deadline for returning the form, but we won’t be able to finalize your application without it. The sooner you’re able to get it back to us, the sooner you can move on and pursue your dream.
